Forefoot drop or forefoot drop is characterized by the disappearance of the arch that runs transversely under the foot, so that the forefoot becomes flat against the ground. This can cause pain when standing, walking and running as the bones of the forefoot are pressed against the ground by the weight of the body, and nerves in the forefoot can get pinched.
Symptoms of a sunken forefoot are usually increasing pain, hard and tender skin on the pads of the foot next to the three middle toes and, in the beginning, often a sleeping sensation in the foot. The sleeping sensation is due to the onset of a pinch in the foot.
Often the initial treatment will be to use an insert in the shoe, a so-called forefoot pad. The insert has a built-in cushion that supports the forefoot and mimics the arch that the forefoot has lost. However, this is not always enough to alleviate foot discomfort in the long term. In these cases, surgery may be required.
Before surgery, you will be examined by a specialist. An X-ray will be taken and based on the images, the doctor will choose the surgical technique that is best suited to your case. In some cases, a CT scan of the foot is also needed, which the doctor will order immediately, but there may be a waiting time for the examination.
During surgery, the surgeon shortens the bones of one or more of the middle toes to remove pressure on the ground.
Around 80 percent of patients who undergo surgery are satisfied with the results of the surgery and have their pain reduced or eliminated. However, there will often be swelling and some tenderness of the forefoot for up to 6 months after surgery. In addition, in many cases, the operated toes are slightly lifted off the ground and the toes may become slightly stiff in the metatarsophalangeal joint.
