Hemorrhoids

Hemorrhoids can be a major nuisance

Hemorrhoids are dilations of blood vessels and mucous membrane/skin just inside the rectal opening or in the rectal canal. It can cause symptoms such as itching, burning, pain, oozing of a small amount of fluid or fresh blood and, in some cases, spotting, where blood appears in large areas of the toilet during bowel movements.

In mild cases, hemorrhoids can be neither seen nor felt, while larger hemorrhoids can be pushed out of the rectum during bowel movements. In severe cases, there may be a constant prolapse of hemorrhoids through the rectal opening. A prolapsed hemorrhoid can become trapped and cause severe pain and swelling. Hemorrhoids can also cause problems cleaning yourself after using the toilet.

Causes and prevention of hemorrhoids

Hemorrhoids can occur for many different reasons and it's not always possible to point to a specific cause. Prolonged constipation, pregnancy and prolonged standing and heavy work can be triggers. In some cases, diarrhea can also cause hemorrhoids due to frequent trips to the toilet.

It's important to keep your bowel movements light to prevent constipation in the future. One way to do this is by drinking 2-3 liters of fluids a day, and over-the-counter products such as Magnesia can be beneficial if you periodically experience problems with hard stools.

Treatment of hemorrhoids

In mild cases, hemorrhoids can be treated with laxatives, ointment, suppositories, lifestyle changes or elastic bands. If there is still a lot of persistent discomfort, surgery may be an option.

At Capio Privathospital, you can have hemorrhoids treated with hemorrhoids:

Hemorrhoid surgery

Surgery with THD procedure (Transanal Hemorrhoid Dearterialization).

In this type of surgery, the blood vessels to the hemorrhoids are tied underneath and excess mucosa is sewn up at the same time. The method used for this is called the THD procedure (Transanal Haemorrhoid Dearterialization) and is a mild and relatively painless form of surgery that leaves no wounds.

Before surgery, you will need to have your bowel emptied. You will need to take an enema (Klyx 240 ml) approximately 2-3 hours before your appointment to ensure that your bowel is empty. The enema can be purchased at the pharmacy and instructions can be found on the bottle. This will be discussed with the surgeon. You will also be required to fast at the hospital, which you will receive further information about in your appointment letter.

Miligan hemorrhoid surgery

The surgery is called Milligan surgery and is performed under general anesthesia supplemented with local anesthesia. With general anesthesia, you will be in a deep sleep and feel no pain during the operation. The prolapse of the hemorrhoid through the rectal opening is pulled forward and cut off. The wound is only stitched with a thin thread to reduce pain and bleeding. The wound may subsequently burst open and must heal from the bottom up. All threads used are self-dissolving and therefore do not need to be removed.

Elastic treatment for internal hemorrhoids

By inserting a small plastic scope into the rectum, the blood vessels in the mucosa that supply the hemorrhoids can be identified. Using a suction instrument that we attach to the mucosa, the surgeon places a small elastic band that stops the blood supply to the hemorrhoid. When the blood supply to the hemorrhoid stops, the hemorrhoid withers. In one treatment, we can usually place up to three rubber bands in different places in the rectum. If one treatment is not enough, we can repeat the treatment after 6-8 weeks at the earliest. This may be relevant, for example, if you have many hemorrhoids.

It is important to avoid hard stools in the days after surgery. You can buy Magnesia 500 mg over the counter at the pharmacy, which is a mild laxative. It is also important that you drink plenty of fluids, preferably 2-3 liters a day.
