Impingement is a painful condition

Impingement is a common diagnosis in our shoulder patients and can be briefly described as an entrapment of soft tissues in the shoulder. In impingement, the bursa and/or supraspinatus muscle between the scapular prominence of the shoulder blade and the humeral head of the upper arm is squeezed, often resulting in inflammation. The condition will often make it difficult for the patient to lift their arm above shoulder level, and general use of the arm will cause shoulder pain.

Causes and symptoms

The condition is caused by inflammation/irritation of the bursa between the shoulder's extensor tendons and the scapula. The initial pain is usually insidious and occurs without any particular strain or injury. However, many people with this type of pain usually have a hard physical job where they perform repetitive movements above shoulder height. However, it cannot be ruled out that the pain can also occur as a result of an acute injury. In some people with entrapment syndrome, an X-ray may show that a new bone or calcification deposits have formed on the front of the scapular bone in the shoulder's extensor tendons.

It is common to have the symptoms below, which can vary greatly in intensity from day to day:

  • Pain at the outer edge of the scapula when lifting the arm upwards and outwards
  • Pain on the outside of the upper arm with pain in the humerus
  • Often the pain occurs at night
  • There may be tingling all the way to your fingers or pain in the back of your neck

Impingement is often long-term. The patient is usually first treated with physiotherapy and possibly anti-inflammatory medication or cortisone injections. If the condition has not improved after 3-6 months despite treatment, surgery may be considered.

Surgery for impingement is often performed as a keyhole surgery. The procedure can remove the inflamed bursa and often involves splitting a ligament and removing the bone from the underside of the scapular prominence to make room for the supraspinatus tendon. After this surgery, several months of rehabilitation with a physiotherapist is required. Often it can take 4-6 months to be free of discomfort.

What type of anaesthetic is used?

Full anesthesia is usually used for arthroscopic shoulder surgery, often in combination with a pain-relieving blockade, a so-called Scaleneus blockade. A blockade is a local anesthetic that numbs the nerves in the arm, leaving the entire arm pain-free during surgery and often for several hours after surgery.
